One hand holds the hook, and the other has the yarn. There are several ways to hold yarn. Try numerous methods and find the one that is most comfortable for you.
The most important thing is maintaining constant tension on the working yarn so that your stitches come out evenly. I used the one my grandmother taught me; you can see it in this video.
